By submitting this request, I authorize Telecom Savings (TS) to provide my long distance service using the Wiltel network and to notify my local phone company of this choice for the phone numbers listed on this form. I understand I may choose only one long distance company for each phone number and that the local phone company may charge a small fee for this change.
